Judging from the rc.submit file, you are right: SmartList only checks the Approved: header if $moderated_flag is set to yes. (I haven't got the latest version of SmartList, so this might have changed.)
The following should probably be called a hack (o, and I did not test it; but it is copied from rc.submit, except for the part between the braces):
Create a rc.local.s00 file like this:
:0 Bhfw # contract header and body * ^^Approved:.*$^ | formail -X ""
:0 * $^Approved:.*$moderator_PASSWORD { moderated_flag=yes foreign_submit=yes }
This convinces SmartList to handle this specific mail as if the list were both open and moderated (including removal of the Approved: header, so that your password will not go to the list).
To activate it, go to rc.custom, and: - set moderator_PASSWORD to your password - remove the comment sign at the beginning of the line #RC_LOCAL_SUBMIT_00 = rc.local.s00 # ...
Maybe there is some better solution.
