In this context, you add the Approved: line after the second "empty line", but SmartList expects it either in the header or after the first empty line.
Try adding it to the header (no, I do not know how to do this with Netscape; sorry).

Where do you add the Approved: line? In the header or in the body?
If you add it in the body, then, in the presence of an attachment, it will not be at the *beginning* of the *body* (where SmartList expects it), but at the beginning of the first body part. I.e., there will be the header of the first MIME part before it.
A MIME message with several parts looks like this:
Mail header (with To:, Subject:, Date:, etc.) empty line MIME separator MIME header of first component empty line body of first MIME component MIME separator MIME header of second component empty line body of second MIME component ... MIME separator
